Horizons Health Care Center is a skilled care facility located in Delta County about 13 miles from the city of Delta. It's scenic rural setting provides a view of the Grand Mesa and the feel of small town country living. The facility was built in 1972 and has added an 11-bed rehab wing in addition to a state of the art rehab gym.The facility is on nine acres of rural property and beautifully landscaped lawns. We also have a newly designed 'Courtyard Garden' with raised planters where our residents grow vegetable and flowers for their own eating a pleasure.
